Physical Therapist - Outpatient (Full-time)

Job description

Position Summary: Performs Physical Therapy (PT) evaluation, reassessment and consultation; provides physical therapy plan of care, goals, discharge planning, patient and significant other (SO) education; renders physical therapy services, interventions, use of therapeutic modalities within the practice guidelines of the American Physical Therapy Association (APTA) and other regulatory agencies; and participates in collaborative patient care services.
