Job Title: Family Nurse Practitioner: Location: York, PA: Employment Type: Full-time: What You’ll Champion: - Seeking a Primary Care Family Nurse Practitioner (FNP) to support a Community Health Center dedicated to improving access to care in York County. - Provide continuity of care for an existing and growing patient population. - Manage and support an established patient panel, including individuals receiving Medication‑Assisted Treatment (MAT). - Help establish care for a vulnerable and underserved patient population in a mission‑driven outpatient setting. - Deliver both in‑person and virtual primary care services, supporting flexible access for patients. - Collaborate within a supportive care team consisting of two CRNPs and one physician. - Practice in a team‑based, integrated primary care environment focused on patient‑centered outcomes.
