RN/LSW Clinic Coordinator

We are seeking a compassionate and experienced RN/LSW to join our Home Health Clinic team! This role is responsible for supporting patients through clinical care coordination, assessments, education, advocacy, and resource management to ensure the highest quality of care and overall well-being. The ideal candidate is organized, detail-oriented, and passionate about providing exceptional patient-centered support in a collaborative and caring environment. Minimum Requirements: - Must have a valid Florida Registered Nurse License. - Must hold current Licensed Social Worker (LSW) credentials - Must possess knowledge of laws, regulations, and guidelines pertaining to Senior Living, Home Health. - Minimum one-year supervisory experience in a healthcare setting.
