Environmental Modifications Care Management Associate (EM CMA) The Environmental Modifications Care Management Associate (EM CMA) plays a key role in supporting Long‑Term Services and Supports (LTSS) members by coordinating home modification projects that improve safety, accessibility, and quality of life. In this role, you’ll serve as the primary point of coordination for Environmental Modification (EM) requests—from initial referral through project completion. You’ll partner with Care Managers, contractors, Medical Directors, and members to ensure projects are delivered efficiently, compliantly, and with a strong focus on member satisfaction.
