Job description

Job Duties include the duties for level 1 and 2 MAP Coordinator plus additional Leadership responsibilities. Other duties may be assigned: Level 1 - Serves as liaison between patients and physicians' offices regarding medication assistance programs. - Oversees completion of enrollment forms and follows up on medications requests. - Maintain EMR data and medication lists related to MAP - Ensures that approved patients are re-enrolled as appropriate - Maintains accounts for MAP patients enrolled in foundations, including billing foundation on their behalf as indicated. - Maintains understanding of the manufacturer’s drug programs and foundations available to qualified MAP clients. - Keeps current on health-related community agencies/resources to refer MAP patients and families as needed. - Assists in development and implementation of marketing plans for the promotion of MAP to clients, hospital staff and community. - Represents the Pharmacy Department and Deaconess in appropriate and related community agency activities. - Participates in Department meetings, in-services, and conferences. - Participates in quality improvement activities related to Pharmacy or Hospital as appropriate....
