This role is responsible for conducting hospice face-to-face encounters, providing symptom management, and supporting patients and families receiving hospice and palliative care services in private homes, assisted living communities, and skilled nursing facilities. The NP works collaboratively with the interdisciplinary team—including physicians, nurses, social workers, and chaplains—to ensure compassionate, patient-centered care that aligns with hospice philosophy and regulatory requirements. Key Responsibilities: Patient Care & Clinical Management: Conduct hospice face-to-face visits to determine continued hospice eligibility in accordance with CMS guidelines.
