About the role Mater has an exciting opportunity for an experienced Geriatric Nurse Care Coordinator (CN) to work within the Hospital in the Home (HITH) Nursing Team. They will act as the central point of contact between patients, families, doctors, and community service providers to ensure continuous, person-centred care, reduce hospital admissions, and help seniors maintain independence in their homes. This role is offered as a permanent role working .2 FTE or 2 x 7:36 (hr:mm) shifts per fortnight. The Hospital in the Home (HITH) Geriatric Nurse Care Coordinator is a dynamic and self-directed role that will manage the comprehensive care needs of older adults, particularly those with complex, chronic, or frail health conditions. They will work within the HITH team, which delivers hospital-level care in the safety and comfort of a person’s residence. Using extensive experience, this role will provide comprehensive coordinated care to these patients and work with the existing HITH team to review protocols and support the development of their peers in the geriatric space.
