Department: 37501 AMC Grafton - Acute Care Rehabilitation Status: Part time Benefits Eligible: No Hou rs Per Week: 0 Schedule Details/Additional Information: Physical therapist in acute care at Aurora Medical Center Grafton with minimum monthly shift requirements as well as 1 weekend shift monthly and 1 holiday per year. Flexible days and hours. Patient populations to include: orthopedics, cardiac, neurological, ICU, post-surgical, general medical. Will receive an additional $3.25 hourly. Pay Range: $41.10 - $61.65 Major Responsibilities: - Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods. - Establishes a plan of care and implements individual treatment based on evaluation results and functional goals. - Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs. - Plans for and completes patient discharge including providing written home programs, patient/fa...
