Inspire health. Serve with compassion. Be the difference. Job Summary: Promotes optimal social functioning of the participants through direct and indirect services. Ensures that while participants are enrolled in the Prisma Healthy Start program, client will have help in dealing with social and emotional problems which interfere with his/her ability to respond to healthcare treatment. Supervise and staff with Resource Mothers (Paraprofessionals) to assist in the development, organization, and evaluation of the Social Work program. Works as part of an outreach team. Be the difference Supervises paraprofessionals and develops Service Plans/Treatment Plans to ensure participants are receiving proper care and services. Recruits and admits potential participants from the community to participate in the Prisma Healthy Start Program.
