The nurse guides members/families toward resources and facilitates interaction with them. These resources are appropriate for the care and wellbeing of members. The Field Care Manager Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. The Field Care Manager Nurse 2 employs a variety of strategies, and techniques to manage a member's physical, environmental, and psycho-social health issues. Resolve barriers that hinder care. Ensure patients are progressing towards desired outcomes by managing patient care through assessments and evaluations. May create member care plans. Understand department, segment, and organizational strategy and operating goals, including their linkages to related areas. Regarding own work methods, occasionally in, and receive guidance where needed. Follow established guidelines/procedures.
