MO-SSM Health St. Clare Hospital - Fenton Worker Type: Regular Job Summary: Provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns. Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES - Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable. Documents medication related plans and interventions in electronic medical record. - Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
