As a Case Manager-RN you are responsible for facilitating resident admissions and discharges, ensuring seamless transitions through their journey. You will work with families, residents as well as internal/external partners to ensure residents receive high quality care and services. Responsibilities: - The RN conducts pre-admission assessment of prospective residents. - The RN develops and ensures continuity of dialogue with hospitals/social workers in community and outlying services regarding availability within community.
