Description Arms Acres is looking for novel clinicians who are ready to change lives in our licensed facility located in Carmel, NY. We provide outpatient treatment, with cutting edge support for those struggling with substance-use-disorder (SUD). An Arms Acre clinician provides comprehensive clinical case management services that include assessments; individual, group and family treatment; treatment plan coordination; discharge planning; and intervention with client support systems, including social service programs and managed care. 5,000k Sign-on bonus eligible RESPONSIBILITIES: - Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures. - Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames. - Documents patient progress and own interventions in case record. Need to remain within time frames of regulatory requirements. - Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director. - Performs utilization rev...
