Treatment will be provided in-person at the CU Medicine Family Medicine – Centerfield clinic. The Licensed Clinical Social Worker will provide short-term psychotherapy across the continuum of care, including identifying eligible patients based on defined criteria, providing psychosocial assessment, developing and executing treatment in alliance with patient’s goals, and ultimately ending care with a firm discharge plan in place.
