Employment Type: Full time Shift: Rotating Shift Description: Responsible for oncology infusion support, inpatient operations, and clinical staffing. The Clinical Pharmacist is responsible for providing clinical pharmacy services in an outpatient and/or inpatient practice area, by providing drug therapy management and assessment for patients in the practice area. Provides support for established programs including, but not limited to, drug formulary, pharmacy education programs, and quality improvement and research relating to drug therapy. What the Clinical Pharmacist will do: Provides drug therapy management and assessment. Identifies and resolves potential and actual drug related problems. Assists physicians in designing dosing regimens based on patient-specific dosing parameters. Communicates and collaborates with physicians and other clinicians to develop drug therapy policies, protocols, and guidelines. Provides support to the Pharmacy & Therapeutics Committee, which may include serving as a member and/or activities such as preparing formulary drug reviews, medication usage evaluations, policies and guidelines for drug therapy, etc. Participates in performance improvement ini...
