Job description

In collaboration with the Keck Referral Center interdisciplinary team, the Referral Services Specialists provides care coordination services evaluating options and services required to meet an individual's health care needs to promote quality outcomes. The Referral Services Specialists works collaboratively with members of the Keck Referral Center team to assess the needs of an individual and family, facilitate the development of an interdisciplinary plan of care, and coordinate care across service sites. The Referral Services Specialists directs the admission of patients and manages the entry process of the patient entering USC Care Ambulatory Services. Ensures appropriate transcription of incoming referrals, collection of medical records and minimum data sets, and completes other duties as assigned. Essential Duties: - On a daily basis, reviews all incoming referrals to initiate and maintain positive working relationship with patients and referring physicians. - Transcribes all incoming referrals into Ambulatory Referral Management - Determine desired means and frequency of communication with patients, i.e., emails, mailed communications , phone calls. - Daily review of referrals...
