Description ACCOUNTABILITIES: - Assesses for immediate and long-term service needs - Completion of monthly observation and monitoring. - Conducts and Completes Treatment Plans. - Assist residents with navigating social service systems and accessing benefits, including Social Security (SSI/SSDI) and other relevant programs. - Obtain and maintain SOAR (SSI/SSDI Outreach, Access, and Recovery) certification. - Planning, scheduling, and coordinating monthly meetings with residents and stakeholders. - Responding to client changes in needs, completing/ submitting appropriate paperwork. - Researching, accessing, and linking to services. - Communicating and working with providers to ensure appropriate service provision. - Meeting with supervisor for weekly supervision and consultation. - Attending/Participating in staff meetings with RA’s. - Assist with formal and informal training of residential staff according to policy and funding source as needed. - Monitor the quality of residential programs through established reviews. - Complete application for social security and other benefits.
