The physician will lead a team of Nurse Practitioners who are embedded in skilled nursing facilities through the region. The physician will be a key part of our structured, collaborative care model that is transforming the delivery of healthcare in post-acute care settings and that improves clinical outcomes and patient experience. You will work at your own pace and have the support of a team of advanced care providers and administrative staff.
