Key Responsibilities: - Conduct comprehensive resident interviews and assessments to identify medical needs and document findings for physician review. - Provide nursing services in accordance with state assisted living standards and the Nurse Practice Act. - Participate in pre-admission assessments for prospective residents to determine appropriate levels of care. - Complete admission and ongoing functional and mental health assessments. - Develop, review, and update individualized service plans based on assessments and changes in resident condition. - Administer and document medications following the “Five Rights” of medication administration, and provide prescribed medical treatments as ordered. - Monitor residents’ health status, obtain vital signs, and promptly report condition changes to physicians, supervisors, and healthcare proxies. - Support residents wit...
