ESSENTIAL JOB FUNCTIONS: Performs tasks pertinent to age of patient (infancy to geriatric). At the written order from a CVHS Provider, makes outside referral appointments. Serves as a liaison between providers and patients for outside referrals. Maintains a Referral Data Base. Prints out a referral list each day. Follows a tracking system by following up on all referrals. Provides written documentation of appointments kept or not kept.
