Department: 39115 Atrium Health Hospice Union - Hospice: Inpatient Status: Full time Benefits Eligible: Yes Hou rs Per Week: 36 Schedule Details/Additional Information: 12-hour shifts, every other weekend, Holiday rotation of all holidays. This position is Sign-On Bonus eligible, up to $15,000.00 for qualified applicants. Internal teammates are eligible for a $7,500.00 Referral Bonus. Pay Range: $35.50 - $53.25 Essential Functions: - Collects data pertinent to the healthcare consumer's health or the situation. - Analyzes the assessment data to determine the diagnoses or issues. - Identifies expected outcomes for a plan individualized to the healthcare consumer or the situation. - Develops a plan that prescribes strategies and alternatives to attain expected outcomes. - Implements the identified plan, coordinates care delivery, and employs strategies to promote health and a safe environment. - Evaluates progress toward attainment of outcomes. - Delegates elements of care to appropriate healthcare workers in accordance with any applicable legal or policy parameters or principles.
