Hours: Temporary full time for up to 4 months What you’ll be doing: - Conduct a comprehensive assessment for the client relevant to biopsychosocial history, diagnosis and ability to make effective use of treatment - Integrate the biopsychosocial history into the treatment plan - Develop and evaluate treatment plans in collaboration with the clinical team, community supports and families. - Facilitate group counselling sessions - Provide individual counselling in support of patient’s specific needs and goals - Ensure continuity of care is maintained through the transfer of information - Work with the clinical team to initiate community treatmen...
