Position Overview: We’re urgently seeking Disability Support Workers to provide dedicated support to a client with Alzheimer’s in Aspley, QLD. Key Responsibilities: - Develop and implement personalized support plans to meet individual client needs. - Provide assistance with daily living skills, community engagement, and education/employment support. - Offer mental health support and guide clients toward greater self-sufficiency. - Assist with social activities, transport, personal care, and other needs as required. - Maintain accurate documentation and case notes to ensure quality service delivery.
