JOB SUMMARY: The Community Liaison interfaces with hospitals, skilled nursing homes, board and care homes and outpatient discharge planning services, and physician offices. Provides and acts as a community resource and public relations representative in facilitating safe quality healthcare in our communities. The Community Liaison is expected to generate appropriate patient referrals/ admissions by establishing and maintaining professional relationships with all referral sources. DUTIES & RESPONSIBILITIES: Generate business by creating new and maintaining existing relationships with physicians and other medical professionals, community and senior organizations, hospitals, appropriate special interest groups, discharge planners, nursing home professionals, assisted living facilities, and other potential referral sources. Contacts prepares and conducts sales and educational presentations for physicians, businesses and local community groups regarding home care services and benefits.
