We believe that every senior deserves to "Celebrate life and wellness to the fullest, Responsibilities: - Follow the service plan in providing services to residents and assist with their medical problems. - Assist in the development and updating of each resident’s service plan. - Assist with services needed to the residents (ADL's). Give residents prescribed medications; and/or remind them to take medications. - Monitor health safety and well-being of the residents and communicate with the Registered Nurse and/or physician when needed. - Document information pertaining to changes in residents' functional status, personal needs, appointments, etc. in the communications log and update the service plan and daily care log sheets if indicated. - Assist in keeping the environment safe for residents. - Provide one-on-one support/visitation to assigned residents on a weekly basis or as requested by the Registered Nurse or as specified in the assistance plan or daily communication log. - Report concerns about residents to t...
