Position Status: Part time Shift: Second Shift (Evenings - Less than 12 hours per shift) (United States of America) Hours per week: 8 Job Information Exemption Status: Non-Exempt A Brief Overview: The Health Connections Staff Nurse assumes responsibility and accountability for a group of patients for a designated time frame and provides care to these patients. Using the nursing process, the consultation nurse facilitates access to the health care system and/or negotiates a satisfactory resolution to the client's problem. The Health Connections Staff Nurse is accountable for quality assessment, efficient managed care, and client satisfaction in collaboration with the health care team. The HC nurse gathers patient information, health history, verification of orders, and scheduling if needed for outpatient procedures including surgical procedures and documents all information within EPIC. She/he is also responsible to make outbound calls for patients with outpatient procedures to assess outcomes, reinforce discharge instructions, and patient satisfaction.
