Become a part of our caring community and help us put health first As a Primary Care Physician with CenterWell, you'll work onsite at our Powdersville clinic and report to the Lead Physician. You'll join a supportive, team‑based environment alongside nurse practitioners, pharmacists, care coaches, behavioral health specialists, and others—all collaborating to deliver personalized, high‑quality care. Main Responsibilities: - Provide high‑quality primary care aligned with clinical and value‑based standards. - Collaborate with the integrated care team to enhance patient care. - Manage referrals, care transitions, DME, and home health services. - Document care and ensure regulatory compliance. - Support daily huddles, on‑call rotations, and center programs.
