Position Summary Under general administrative direction, provides a significant level of patient education related to illness and planned management and treatment and facilitates coordination of care for INFECTIOUS DISEASE patients. In addition, the Nurse Navigator provides in-clinic support to providers, as needed. Essential Duties and Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned. - Liaison for ID patients requiring coordination of care and support. - Work collaboratively with the administrative and clinical ID teams to manage clinical issues; urgent tasks, prescription pre-authorizations and refills, appointments. - Coordinate and manage all urgent orders for dynamic testing and procedure referrals to include patient preparation and education. - Follow up with patients on testing and procedure results in coordination with providers. - Provide appropriate clinical support, counseling, and education.
