Description Provide consultation services to participants and/or their representatives accessing Community First Services and Supports (CFSS) through a person-centered approach that promotes self-direction and reflects the core Principles of Self-Determination. This includes supporting individuals to make meaningful decisions about how they want to live their lives, exercise authority over a designated budget, and organize resources in ways that enhance their quality of life. Responsibilities also include fostering accountability in the use of public funds, recognizing the valuable contributions individuals with disabilities bring to their communities, and affirming the essential role of self-advocates in shaping and navigating a redesigned support system. Services include CFSS orientation and training, ongoing case management, assistance with service plan development, and facilitation of connections to external resources and service providers.
