Description The Triage Nurse is responsible for delivering high-quality nursing care under the direction of a supervising provider or clinical team, ensuring that patient and family needs are effectively met. This role involves collecting and analyzing data to support patient assessment and care planning, including triaging patients, managing telephone communications, and reviewing medical documentation such as clinical and hospital records. The Triage Nurse prioritizes tasks efficiently, processes medication refill requests, and ensures that care delivery aligns with established care plans and office policies. Responsibilities: - Provides nursing care to patients as delegated by assigned supervisor and/or provider to meet the needs of the patient/family - Collects data and information to assist the clinical team and/or the provider in patient assessment and care planning in accordance with current MIPS guidelines which includes but is not limited to: - Triaging patients - Performing real-time clinical assessments and EMR Documentation during visits - Communicating internally & externally through telephone encounters & actions - Supporting providers with in-room needs - Vitals, ECG...
