Minosa Case Manager

Job description

This role provides intensive case management support to men experiencing homelessness or at risk of homelessness within a crisis and transitional accommodation setting. Working from a client-centred and trauma-informed approach, you will engage with residents to assess their individual needs, develop tailored support plans, and assist them in achieving stability and independence. You will support clients to access housing pathways, health and mental health services, income support, and other community resources, while fostering a safe, respectful, and inclusive environment. The successful candidate will build strong, professional relationships with clients, empowering them to set and work toward personal goals. You will collaborate with internal teams and external service providers to coordinate care and advocate on behalf of clients, ensuring continuity of support. Strong communication, risk assessment, and problem-solving skills are essential, along with the ability to manage complex and sometimes challenging situations with empathy, professionalism, and resilience.
