The University of Miami Health System, "UHealth", Department of Pediatrics- Early Steps, has an exciting opportunity for a full -time Case Worker 1. The Case Worker assists patients in keeping appointments, identifies barriers to care, coordinates the development of a treatment plan, and monitors the implementation and follow-up of treatment/service provided or ordered by health care providers. Core Responsibilities: Conducts patient evaluations to obtain information on the patient's medical and mental health, treatment and recovery to develop an effective treatment plan. Ensures patients are enrolled in insurance and free or low-cost social service programs for which they are eligible (e.g., food stamps, WIC, housing, SSI, Medicaid/Medically Needy Program, Ryan White, and ADAP, etc.).
