She/he ensures compliance with Nurse Next Door’s operating policies and procedures. The Care Designer supports the Franchise Partner in the oversight of caring practices, supervision of care and the delivery of responsive service. The Care Designer will assume on-call responsibilities as assigned. ESSENTIAL DUTIES & RESPONSIBILITIES: Client Care Management: - Develops, promotes and ensures high quality, innovative customer service; - Conducts caring consults and assesses clients with respect to physical, psychosocial, and environmental needs; - Develops appropriate and individual care plans and updates as required; - Ensures that appropriate kinds of intervention take place to meet the client’s needs and rights; - Liaises with other agencies involved with the client to ensure the provision of integrated services; - Collaborates wi...
