Key Responsibilities: - Evaluate patients’ speech, language, and hearing abilities, recommending strategies to enhance communication and hearing. - Perform initial and ongoing patient assessments, including OASIS evaluation and documentation. - Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed. - Communicate regularly with physicians, addressing any changes in patient condition. - Participate in the periodic revision of care plans, updating the physician and care team on patient status and obtaining additional orders as required. - Educate patients and families/caregivers on speech therapy techniques and related challenges. - Confirm daily visit schedules to ensure seam...
