This role ensures clients receive coordinated, recovery-oriented care, helping them access resources, overcome barriers, and achieve their treatment goals. ESSENTIAL JOB FUNCTIONS: Educate and refer patients to community agencies and businesses that appropriately address the patients’ needs for food, clothing, shelter, employment transportation, etc. Utilize and manage Electronic Health Record (EHR) systems for data entry and client information tracking, ensuring program alignment.
