Job description

24 hours per week Monday- Friday: Dayshift position (8-hour shifts) Job Summary: Responsible for the operation and services provided within the specialty medical practice or remotely through centralized telephonic clinical support. Delivers a high level of service to each patient in every interaction, ensuring they perceive Baptist Health Deaconess Madisonville’s pharmacy as the pharmacy of choice and ultimately impacting their level of success in their therapy. Provides clinical consultation, chronic disease state management, medication reconciliation, and adherence counseling for specialty patients. Plays an integral role on the interdisciplinary team including pharmacists, billing specialists, nurses, medical assistants, and providers. Facilitates the development, implementation, and maintenance of programs that promote desirable patient outcomes through safe, appropriate, cost-effective medication therapies. Ensures compliance with pharmacy regulatory standards.
