Responsibilities: The Resource Specialist will provide Care Management services to patients who have been identified by their primary care provider or staff members at Legacy Clinics as needing ancillary mental health / psychosocial services. Key services include (but are not limited to) psychosocial assessment; facilitating access to community resources; coordination with community organizations on behalf of patient; crisis management; access to medications and insurance; transition planning; education. Works as part of multidisciplinary team which may include physicians, pharmacists, clinic providers and staff, and other Care Management staff.
