Our Company Adoration Home Health and Hospice: Overview: Under the general administrative direction of the reporting manager, the E-Referral Specialist will receive and respond to community, physician, patient and family request for home health. This position will record and process referral information as well as document in patient information database. The E-Referral Specialist will schedule registration and admission visits, prepare all required paperwork for admission as well as act as a liaison between patients, families, referral sources, physicians and admission staff. They will also assist in distribution of admission information and report to primary teams and physicians. Responsibilities: - Perform required data entry accurately at the times specified by the department. - Patient information and forms are managed and forwarded to the appropriate department per procedure. - Monitor all assigned portals for new referrals, messages, bookings, and updates. - Work with the intake department and local branches to ensure communication of all booked referrals, referral sources messages and updated documents coming through the portal and to minimize any delays in care of our pati...
