Job Purpose & Position Overview The Outpatient Clinical Nurse, RN is responsible for objectively assessing, planning, implementing, and evaluating nursing care in coordination with a multidisciplinary team to provide “patient centered” care. The Registered Nurse” will assist in the ongoing management of the patient based on identified psycho-social needs, continuity of care, disease prevention, chronic disease management, and provides patient education to promote Health Literacy. Qualifications/Skills: - Demonstrate the WCHS core mission, vision and values; respecting tribal beliefs, culture, and traditions - Formulate clinical decisions that demonstrate critical thinking to meet the physical and psychosocial needs of the patient through collaborative practice with the healthcare team. - Completes comprehensive initial assessment and completes documentation accurately and completely in the patient record. - Demonstrates expertise in systems assessment, recognition of significant changes in patient condition, establishment of priorities, and nursing interventions. - Provides interventions as ordered by the Medical Doctor, Nurse Practitioner, Physician Assistant - Evaluates patient r...
