Job Summary The Care Coordinator II is responsible for coordinating multiple aspects of the youth’s and/or families’ care. This role provides the coordination and delivery of high-quality psychosocial care to youth and families. The Care Coordinator II works with other care providers and community partners to ensure that the overall needs of the client(s) are met. The Care Coordinator II may also be assigned to lead projects or mentor program staff. This is a full-time (40 hours) position. Primarily on a Monday-Friday day shift schedule, but requires flexibility for afternoon/evening hours based on client need. Additionally, the hire will need to be on the on-call rotation after the initial training period.
