Job description

Overview The medication history pharmacy technician assists pharmacists in providing care to inpatients and outpatients of all ages by collecting information to compile an accurate and complete list of medications a patient was taking prior to admission and helps the pharmacist in any routine duties not requiring professional judgment. Responsibilities: - Interviews patients and their families for the purpose of compiling an accurate and complete list of medications the patient was taking before admission. - Accesses pharmacy claims data and contacts the patient’s community pharmacy, physician’s office, or other healthcare facility for clarifications if needed. - Works closely with the supervising pharmacist and nursing staff to coordinate medication history assessment - Verifies patient allergy information and ensure complete documentation - Documents the medication history and allergy information in the electronic medical record accurately and in a timely manner. - Reports suspected medication errors or adverse drug reactions to the supervising pharmacist - Is able to perform duties of a pharmacy technician - Systematically checks any assigned pharmacy stock areas for removal of...
