Description JOB SUMMARY: This position will split time between Assistant Community Support Coordinator duties and Medical Services Associate duties for a total of 40 hours per week. Under the direct supervision of the Area Director, the Medical Services Associate provides assistance to individuals supported by Apace by identifying, scheduling, and attending physical, mental, or behavioral health appointments and ensuring accurate record-keeping and effective communication with providers, Apace staff, family and/or guardians to ensure healthcare needs are met. The Assistant Community Support Coordinator, under the direct supervision of the assigned Community Support Coordinator (CSC) or Area Director, provides support and assistance in the supervision of staff and activities involved with training, health and well-being, and the enhancement of persons with developmental disabilities through the implementation of the Individual Support Plan (ISP). The Assistant Coordinator may also substitute for the direct support staff, Direct Support Professional (DSP), as needed.
