Transitional Case Manager, Outreach Direct Service (OPSEU) Full-time, Contract (End date: January 23, 2027) This posting is the result of a current vacancy. London, Ontario (London Zone): TCMs develop strong working relationships and collaborative partnerships with other providers including but not limited to LHSC, addiction services, and social supports. TCMs focus on assisting individuals to gain skills to achieve their recovery goals, improve overall health and well-being. This will be done through a variety of means which may include referrals to other internal or community programs, connection to addiction and mental health supports, psycho-social education, introduction of coping strategies, crisis planning support, and goal planning.
