Altus Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community. Schedule: Monday - Friday, 8 am - 4:30 pm with On Call Expectations One Night/Week Territory: Pasadena and Baytown Area I-10 East In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.
