Job description

Responsibilities: - Conducting pre-enrollment assessments for prospective enrollees utilizing clinical nursing expertise and critical thinking skills including obtaining preventive health service data. - Completes UAS - New York Assessment instrument to determine continuing eligibility and / or identify any needed changes to Care Plan. - Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and will need to develop a prospective plan of care. - Communicates with the Care Team and participates in interdisciplinary team meetings. - Assessments will be conducted in Westchester. Reliable transportation required. - Responsible for environmental assessments. - Completes semi-annual assessments as requested - Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities. - Conducts chart audits as requested if time permitting Education: - Current NY State Registered Nurse License - Bachelor’s Degree in Nursing
