Social Worker POSITION SUMMARY: The Social Worker functions as a part of the multidisciplinary care team to plan, organize, and implement social work services for patients and families based on individualized needs. The Social Worker performs these functions consistent with the Utilization Review and Quality Improvement Plans with a patient-focused function to promote the restoration of health and the achievement of patient health goals. RESPONSIBILITIES: - Perform psychosocial assessment of Social Work High-Risk Criteria cases. - Assess and assist patients and families to identify discharge barriers. - Work with an interdisciplinary team to develop a discharge plan for complicated psychosocial issues that are identified. - Provide information and referrals based on community resources and within patient insurance coverage. - Serve as a liaison between community agencies and hospital. - Supervision of profession social work students when appropriate. - All other duties assigned.
