Posting Date 04/23/2026 8110 Network Dr, Plainfield, Indiana, 46168-9024, United States of America No On-Call: The primary work location is at the DaVita den on-site at Majestic of Southport located at 8549 Madison Ave Indianapolis, IN 46227. This den has experienced an increase in patient census and now operates Mon-Saturday. This person must be flexible to support hours at other local DaVita SNF Dialysis or DaVita In-Center Hemodialysis locations as needed. DaVita SNF Dialysis teammates provide dialysis to patients on-site at Skilled Nursing Facilities in a 6-chair den and have access to all the resources DaVita has to offer. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week, placing a significant burden on patients and adding significant avoidable costs to the health care system. Ideal teammates will have experience in dialysis, get energized by a start-up type environment and improving the lives of patients, and have a track record of finding ways to Get Stuff Done (GSD).
