Residential Case Manager Position Summary: The Case Manager performs case management-related functions to coordinate, facilitate and maintain care for the residents of Venture House and the Apartment Treatment program in Madison County. Residential Case Manager Responsibilities: - Completes assessment of housing and housing related needs. - Works with Venture House and Apartment Treatment staff, and multi-disciplinary treatment team to establish individualized service plans, which meet the clients’ goals. - Establishes and maintains effective relationships with other service providers: e.g. Madison County DSS, SPOA Coordinator, PROS,...
