Job description

The RN Field Case Manager ensures and maintains the highest quality of care for our clients through our Care Professional workforce while building and maintaining excellent relationships with all external clients, customers, and field staff. Responsible for optimal clinical and financial outcomes, as well as stakeholder satisfaction, for an assigned caseload of clients. This role is primarily field based with most work time spent in client homes and the community completing assessments, supervisory visits, and client/care professional support. Responsibilities for this role includes: Clinical Assessment & Care Planning Field Clinical Oversight & Care Professional Support - Completes and documents initial assessments, reassessments, and significant change in condition assessments for new starts, restarts, and ongoing cases in the EMR per company standards and applicable state requirements. - Develops, implements, and updates individualized plans of care based on assessed needs, client/family goals, and provider direction. - Identifies clinical risks and changing conditions; initiates timely interventions and escalations to the appropriate provider, family, internal leadership, or em...
