We don’t just deliver services - we walk alongside our Elders to ensure they are supported in a way that respects who they are, where they come from, and what matters to them. About the Role: This role sits at the heart of how we support our Elders to remain independent, safe and connected at home. As a Care Partner, you’ll work closely with clients to understand their needs, goals and what matters to them, and translate that into clear, practical care plans that support independence and wellbeing. The role focuses on care planning and case management - actively monitoring how clients are going, identifying changes early, and making sure supports remain appropriate, effective and aligned to their goals and available funding. You’ll build trusted relationships with clients and their families, providing guidance and continuity, while working alongside the broader team to ensure services are delivered in a way that makes sense for each person.
